1. List recurring decisions about scope, design, data, access, budget, timeline, adoption, and acceptance.
2. Assign one accountable owner and the people who advise, execute, or must be informed.
3. Set response windows and the escalation path for decisions that threaten a milestone.
4. Keep a dated decision log with options, evidence, owner, outcome, and downstream effect.
